PERSOL is seeking a dedicated Public Healthcare Care Coordinator to join our team and manage the Asthma Program remotely from Bali. This role is crucial in ensuring the effective implementation and monitoring of our asthma care initiatives, aiming to improve the health outcomes of our target population.

As a Public Healthcare Care Coordinator, you will play a pivotal role in coordinating healthcare services, managing patient data, and ensuring the delivery of high-quality care to those in need.

📋 Tanggung Jawab Pekerjaan

Tanggung Jawab:

  • Coordinate and manage the Asthma Program, ensuring its effective implementation and monitoring.
  • Develop and maintain patient care plans, ensuring they are tailored to individual needs.
  • Conduct regular patient assessments and provide necessary care and support.
  • Monitor and evaluate program outcomes, providing regular reports to management.
  • Collaborate with healthcare professionals and community leaders to ensure the success of the program.
  • Ensure compliance with all relevant healthcare regulations and guidelines.
  • Provide training and support to healthcare staff involved in the program.
  • Maintain accurate and up-to-date patient records and data.

    Kualifikasi:

  • Bachelor's degree in Public Health, Nursing, or a related healthcare field.
  • Minimum of 3 years of experience in public healthcare or a related field.
  • Strong understanding of asthma care and management.
  • Excellent communication and interpersonal skills.
  • Proficiency in English, both written and spoken.
  • Familiarity with healthcare software and data management tools.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
